New Patients

Patients living within the practice area are welcome to register and our reception staff can confirm your eligibility from your postcode. In this respect please bring along proof of address such as a recent utility bill as well as your medical card if you have one.

You will also need to complete a registration form (GMS1) and a health questionnaire which will provide useful information whilst we wait for your medical records to arrive from your previous doctor.

All new patients are offered a health check with a member of the healthcare team.

Medical treatment is available from the date of registration. Please contact reception for further information.

From 20th October 2014 we are unable to accept patients outside our boundary.

No patients from Chatham, Gillingham or Rochester area will be accepted.

Register Online

If you wish to pre-register please fill out our online Patient Registration form. When you visit the surgery for the first time you will be asked to sign the form to confirm that the details are correct.

When you register you will also be asked to fill out a medical questionnaire. This is because it can take a considerable time for us to receive your medical records. When you come to the surgery you will be asked to sign this form to confirm that the details are correct.
